The Inspection Service goes to Italy to find the real macaroni and they succeed. Our macaroni turns out not to be good macaroni and tastes like cardboard. Real macaroni also has a hole, but it certainly doesn't have to be in the shape of an elbow. Of course, spaghetti doesn't have that hole.
